چکیده
Letters an unusual case of localised hypertrichosis editor Topical testosterone is now a widely used mode of testosterone replacement therapy. It is well reported that transcutaneous absorption of testosterone may lead to hirsuitism and virilisation. 1,2 We report an interesting case of localised hypertrichosis on the forearm of a female patient and postulate that this was the result of accidental transfer of testosterone gel from the patients' husband. A 66-year old lady presented with a one-year history of localised hair growth on the right forearm. She denied excess hair growth or alopecia elsewhere and did not report any other signs of virilisation. The patient had no other relevant past medical or drug history. Closer questioning revealed that she had been applying a 5 % testosterone gel (testogel ®) with her right hand to her husbands shoulder, intermittently for 4 years. This was applied daily for hypoandrogenism, secondary to radiation therapy for multiple myeloma. On examination she had localised hypertrichosis on the right forearm, sparing the right dorsal hand, associated with an eczematous eruption. (Fig.1) There were no other relevant clinical findings. Hormone profile including free testosterone was normal. In view of the temporal relationship between application of the gel and the findings of localised hypertrichosis with a normal hormone profile, we feel that the intermittent application of testogel ® was the causative factor. In relation to topical testosterone, there have been recent case reports of precocious puberty in children and hirsuitism +/-virilisation in women following accidental transfer of topical testosterone. 2 One recent case report and review of the literature also described progressive hirsuitism in a pre-menopausal woman associated with fluctuating testosterone levels of 1.6-6.7 over a 3-month period (normal range<2.5). 2 This was felt to be secondary to transfer of testosterone gel from her partner during contact, because her hair growth and testosterone levels returned to normal after her partner switched to injectable testosterone. 2 Not all cases are associated with hyperandrogenism however. In one case series of two females applying testosterone gel for treatment of lichen sclerosus et atrophicus, both developed hirsuitism two months later. 1 Hormonal profiles were normal in both cases, however, the gel had been discontinued several weeks before presentation. 1 Localised hypertrichosis is another known side effect of topical testosterone gel. In a recently published study looking at the effect of transdermal testosterone in female patients, the investigators found that the commonest side effect …

Pancreatic and Gastric Heterotopia with Associated Submucosal Lipoma Presenting as a 7-cm Obstructive Tumor of the Ileum: Resection with Double Balloon Enteroscopy
Pancreatic and gastric heterotopias are rare clinical entities which have been identified throughout the entire length of the gastrointestinal tract. Combined gastric and pancreatic heterotopias, although unusual, have been described in the duodenum and jejunum, and in other structures, including Meckel's diverticulum and the ampulla of Vater. We report a novel case of pancreatic and gastric he...
متن کاملEndoscopic submucosal dissection of pancreatic heterotopia in children.
Aberrant pancreatic tissue is mostly found in the submucosal layer of the upper gastrointestinal tract, occurring in 1.7% of the population according to summarized post-mortem studies [1]. Gastric pancreatic heterotopia was first recognized by Klob in 1859 [2]. Hereinwe describe the resection of gastric pancreatic heterotopic lesions in children by endoscopic submucosal dissection (ESD). After ...
متن کاملMalignant Transformation in Gastric Pancreatic Heterotopia A Case Report and Review of the Literature
Context Gastric pancreatic heterotopia is a relatively common incidental finding; however, malignancy arising in this tissue is an extremely rare occurrence, with nonspecific clinical, radiographic, and intraoperative features. We herein report the case of adenocarcinoma arising in ectopic pancreatic tissue of the gastric antrum and review the literature to summarize all recent cases of maligna...
متن کاملGastric pancreatic heterotopia: an unusual cause of upper gastrointestinal bleeding.
A 28-year-old woman presented with history of chronic intermittent anemic symptoms with dark stool for few years. Her blood tests showed a low hemoglobin level (6.6mg/dL). She underwent esophagogastroduodenoscopy (EGD), which revealed two subepithelial lesions, diameter 1.0cm and 1.5cm and with central umbilication, located in the antral area. These endoscopic findings typically suggested pancr...
متن کاملSalivary gland choristoma in large bowel.
The occurrence of heterotopic tissue in the large bowel is relatively rare. The most common type of tissue in such heterotopia is gastric mucosa, but rare cases of pancreatic and salivary tissue are also reported in literature. All cases of salivary choristomas reported in literature arose in the rectum–anal canal region [1–5]. Here we report a submucosal salivary gland choristoma in the sigma ...
متن کاملGastric MALT Lymphoma Presenting as a Large Solitary Submucosal Mass
Herein, we report on a 24-year-old woman with a large well-marginated gastric mucosa-associated lymphoid tissue (MALT) lymphoma. She presented with dysphagia and epigastric pain. In esophagogastroduodenoscopy there was a huge bulging submucosal mass with normal appearing mucosa in the cardia in addition to a grade B gastro-esophageal reflux disease. She was treated with a proton pump inhibit...
